Clinical Outreach Specialist

Full Time - 32 Hours per week/64 ppd

SUMMARY OF JOB RESPONSIBILITIES

The Clinical Outreach Specialist acts as a liaison to identified hospitals, facilities, community partners, physicians, medical health professionals, payers, and patients/families and all other referral sources through distribution of education material, in-services, conferences, regular contacts and other marketing activities.  Also works with staff to support and educate on community outreach along with providing internal marketing and support to facility staff.  Plans and conducts public relations activities and marketing initiatives designed to create and maintain a favorable public image for Harbor Hospice and to raise community awareness regarding end of life choices. 

QUALIFICATIONS

  1. Bachelor's degree from a four-year college or university.
  2. Minimum of 2-3years related experience in sales or marketing in the healthcare industry.
  3. Maintains a valid Michigan driver's license and has access to a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.
  4. A demonstrated commitment to creating a culture of excellence with strong customer service and an engaged workforce is desired.

 

Poppen Residence CENA (Two Open Positions)

Full Time 32 Hours per week/64 Hours ppd

Combination of 11:00am-7:00pm Kitchen/Cleaning and 7:00am-3:30pm Patient Care 

SUMMARY OF JOB RESPONSIBILITIES

Provides direct care to patients in the Hospice Residence including, baths, vitals, back rubs, oral hygiene, shampoos, changing bed linen, housekeeping and laundry.Cooks meals in quantities according to menu, standardized recipes, and patient census, within health code, and ensures all food is maintained at appropriate temperature per regulations and ensures utmost quality with a minimum of waste.     

QUALIFICATIONS

  1. A nurse aide training and competency evaluation program approved by the State as meeting the requirements of § 483.151 through § 483.154 of this chapter, and is currently listed in good standing on the State nurse aide registry.
  2. Current State of Michigan Nurse Aid Certification
  3. CHPNA preferred

 

 

Poppen Residence CENA (Two Open Positions)

Part Time 16 Hours  Biweekly  Every Other Weekend

(1)    1st Shift  7:00 am – 3:30 pm  &  (1) 2nd Shift  3:00 pm – 11:30 pm 

SUMMARY OF JOB RESPONSIBILITIES
