Director of Clinical Services


Harbor Hospice, a recognized leader in Hospice and Palliative Care, serving a five-county area along the lakeshore in west Michigan, has an exciting opportunity available for a Director of Clinical Services.  This position provides leadership for the planning, development and delivery of all patient care services and programs including home and inpatient hospice and community palliative care.  An important focus of this position is operational and clinical integration with the larger health system and continuum of care providers.  This is a key position on our senior leadership team and reports directly to the Executive Director. 

The Director of Clinical Services is responsible for the oversight and provision of the organization’s clinical services, including the employment of qualified Hospice personnel.  The Director of Clinical Services establishes, implements, and evaluates the agency’s care, treatment, and services to comply with federal and state regulations; maintains quality standards, Joint Commission accreditation, and promotes the organization’s philosophy of care.  The Director develops strategies for growth and program enhancements in support of the organization’s strategic plan.  He/she considers the organization’s mission and the organization’s business needs in all decision making.  An important focus of this position is operational and clinical integration with the larger health system and continuum of care providers.

This is a key position on our senior leadership team and reports directly to the Executive Director.

Qualifications:

·        Current Michigan RN License
·        Minimum of 5 years of nursing and 3 years of healthcare management
         experience.
·        Bachelor’s degree required Master’s degree preferred.
·        Certification in hospice and palliative care preferred.
·        Maintains a valid Michigan driver’s license and has access to a reliable,
         licensed and insured vehicle for use on the job, vehicle must be
         insured in accordance with the State of Michigan.
·        A demonstrated commitment to and experience with creating a
         culture of excellence, strong customer service, positive clinical
         outcomes, and an engaged workforce is desired.

Intake Nurse 
Registered Nurse
 

Harbor Hospice, a Joint Commission accredited organization, has an opening for a Registered Nurse in our Intake Department. This position is part-time, 40 hours bi-weekly including every other weekend. An example schedule would be Monday & Tuesday 10am to 6pm one week, Saturday and Sunday 9am to 5pm and Friday during the second week.

This position is responsible for patient referrals and intake activity; liaison with area discharge planners, referrers, payers and patients/families.

This position requires 3-5 years clinical experience, strong assessment skills, strong computer charting skills, interpersonal skills and ability to work within a team. Current State of Michigan RN license and valid driver’s license required. BSN preferred with experience in hospice and community care.

  PRN Primary Care

  Registered Nurse 

The RN provides skilled professional nursing care to the patient/family/caregiver as prescribed by the physician and in compliance with regulation as established by the State Board of Registered Nursing, the patient and the policies of Harbor Hospice.

 Qualifications:

  • Is a graduate of an accredited school of Nursing.
  • Current state of Michigan Registered Nurse License.
  • Minimum of 2 years Nursing experience
  • Certification in hospice and palliative care preferred.
  • Possesses and maintains a current CPR certification.
  • Maintains a valid Michigan driver’s license and has access to a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.
  • Must be very flexible and able to work all shifts.
  • Able to work various roles in Home Care.

Poppen Residence CENA

 

Part Time 16 Hours  Biweekly  Every Other Weekend

2nd Shift  3 pm – 11:30 pm

SUMMARY OF JOB RESPONSIBILITIES


Provides direct care to patients in the Hospice Residence including, baths, vitals, back rubs, oral hygiene, shampoos, changing bed linen, housekeeping and laundry. Cooks meals in quantities according to menu, standardized recipes, and patient census, within health code, and ensures all food is maintained at appropriate temperature per regulations and ensures utmost quality with a minimum of waste. 

QUALIFICATIONS 

 

  1. A nurse aide training and competency evaluation program approved by the State as meeting the requirements of § 483.151 through § 483.154 of this chapter, and is currently listed in good standing on the State nurse aide registry.
  2. Current State of Michigan Nurse Aid Certification
  3. CHPNA preferred
